Seeking Help for Maltese With CKD & DKA

Can someone advise me? My little man Maltese, named Buddy, was in the hospital for 4 days with DKA and CKD., he came out a fighter. He is now a diabetic, and they say he has chronic kidney disease. I give him the insulin 2x a day, and he wears the libre to monitor his glucose, and is good. But the vet checked out and doesnt care anymore and says negative stuff and Im not ready to give up.
